Touch-Free Contact Tracing

Google provides a few form templates to get you going, including a basic Contact Information form which has nearly everything you need. There’s a few things I would change about this form:

  • You might want to replace the “Comment” field with a Dropdown field to indicate which class they were taking part in.
  • You could also add a Checkbox field for a declaration that the person checking in is well and is not experiencing respiratory symptoms.
  • Go through and make sure that the name, address and phone number fields are compulsory. These are required by the Ministry of Health. You may want to make other fields compulsory (e.g. what class they were in) to help you keep track of who was in contact with who.

You can change the header image and colour scheme to match your club or business colours by clicking on the little paint palette icon in the top menu bar.

Step 2: Print a QR Code

Once you’re done with your form, click the “Send” button at the top of the screen and choose the second option to get a link to your form. Copy this link.

There are lots of free QR code generators available online. I chose QRCode Monkey because it was the first hit on Google. I pasted in the link to my form and it gave me a PNG file containing my QR code.

Now you can make a poster for the entrance to your training facility, including the QR code so that they can scan it to sign in. Most people will be able to scan this directly with their phone’s camera app. Some with older phones may need to download an extra app to read QR codes. Of course you may also have some dog owners in your class who don’t own a smartphone at all, so you’ll need to fill in the form for these people.

Step 3) Access Your Visitor Log

Fill in your form on your phone, and then go back into Google Forms and choose the “Responses” tab for your form at the top of the page.

This will show you an awesome (but rather well-hidden) button to export your form responses to a Google Sheet. This Sheet will show the time that the form was filled in (albeit with the stupid American date format), as well as all of the data that the user filled in.

It’s a good idea to check the form after every class to make sure that everyone is included. You can now sleep easy, knowing that you have all of the data you need in the unlikely event that it is needed.
